In Providence, Rhose Island, local politician Tommy Caffee finds that his life has become more complicated now that his criminal brother, Michael, has returned home.
Michael tries to take over a business by forcing the mentally-challenged brother of the store's owner to play a game of Russian roulette. Tommy is wheeling and dealing in an effort to stop a new highway from destroying the Hill.
Tommy has to help settle a garbage strike, while his wife, Eileen, comes to an end of an affair. Michael tries to recruit for his new crew, and struggles with the changes his neighborhood has gone through in the seven years that he had been away.
Tension between the Caffee brothers grows when Treasury Agents looking for counterfeit money, raid their mother Rose's house during Sunday dinner. Tommy looks to the Speaker for help on a bill that would benefit his real estate development deal.
Tommy has to balance his loyalty to the Speaker with his loyalty to The Hill. Michael deals with Armenian drug dealers working out of a neighborhood bar, and Rose fights back during the layoffs at the mill.
Tommy and Eileen's marriage hurls towards crisis, as Eileen goes off the rails. Their daughter, Mary-Rose, finds one of her mom's joints and experiments with the drug.
Michael convinces Tommy to help revive an old movie theatre. Eileen, though denying her drug abuse to Tommy, confesses and admits her problems to Mary-Kate.
Michael takes an old girlfriend on a road trip while his mother, Rose, plans a surprise birthday party for him. The House of Representatives gets a new Speaker and Tommy maneuvers to get himself a promotion.
The Hill mourns when a horrific bus crash injures and kills several people coming back from a Patriots game. Everyone is affected by this tragedy, and the mob boss, Freddie Cork, learns that his son was directly involved with the accident.
Tommy and Michael's long-lost father resurfaces and threatens to sell Rose's house if they don't help him out of a financial jam.
A dark cloud looms over the Hill when everyone comes together for a big Irish wedding. Freddie Cork orders a hit on Michael, while a crooked U.S. Attorney tries to nail the Caffee brothers by sending the informant, Marty Trio, in with a wire.
